I am writing to dispute the collection account that has been listed on my credit report. I do not believe that I am responsible for this debt and I request that it be removed from my credit report immediately.

According to my records, I never received any notice or communication regarding this debt prior to it being placed on my credit report. Without this prior notice, I was not aware of the debt and therefore did not have the opportunity to resolve it.

I request that you provide me with documentation and proof that I am indeed responsible for this debt. Until I receive this documentation, I request that you do not report this collection account to any credit bureaus.

I understand that it is the responsibility of the creditor to provide evidence of the debt and that you, as a collection agency, are required to validate the debt upon request. I request that you fulfill this obligation and provide me with the necessary documentation.
